Presbyterian is committed to improving access to healthcare, behavioral health, healthy food, opportunities for exercise, and provide resources based on various social determinants of health. We do this by conducting community health assessments so we can identify the priority health issues facing each of our communities.
Presbyterian Santa Fe Medical Center worked with the Santa Fe County Health Policy and Planning Commission, the Department of Health, and many other community partners in Santa Fe, to complete a community health assessment and identify community health needs.
