Community Health Initiatives

Presbyterian is committed to improving access to healthcare, behavioral health, healthy food, opportunities for exercise, and provide resources based on various social determinants of health. We do this by conducting community health assessments so we can identify the priority health issues facing each of our communities. Presbyterian Santa Fe Medical Center worked with the Santa Fe County Health Policy and Planning Commission, the Department of Health, and many other community partners in Santa Fe, to complete a community health assessment and identify community health needs.

Community Health Programs

Northern Roots

This Presbyterian Community Health program is open to families with children under the age of 18 and to pregnant women.
Upon being referred by a provider, participants receive fresh produce from our partner, MoGro Mobile Grocery, for 16 weeks along with nutrition resources – all at no cost.
Produce distribution takes place every Thursday from 1:00 to 3:00 p.m. at Santa Fe Medical Center.

FreshRx

FreshRx is a produce prescription program in partnership with the New Mexico Farmers’ Marketing Association.
The program provides incentives for patients with conditions such as obesity, heart disease or hypertension to add fresh, locally grown produce to their diets.
FreshRx is offered during the market season and is available upon referral from a provider. Enrolled participants receive monthly vouchers to use when shopping at farmers’ markets.
